Things to Remember Prior to Your Appointment
If required by your insurance company, referral forms and authorizations must be received form your primary
care physician prior to your appointment.
- Please provide 24-hour notice if you need to reschedule an appointment.
- Complete new patient forms prior to arriving for your appointment or arrive 15 minutes early to complete the information.
- Be prepared to provide a urine sample.
- Established patients should notify our receptionist of any changes in insurance, address or contact information.
Emergencies and Prescription Refills
If you need assistance during regular office hours, call our office and you will be given instructions based on the nature of your call. For assistance after office hours call Lake Norman Urology and you will be directed to the physician on call. In the event of a true medical emergency, call 911 or go the closest emergency department.
